Driving licence renewal need help

hi there when i was dx 5 years ago i stopped driving never contacted the dvla.now i have decided i would like to start driving again.i could also do with a photo licence as my old licence is the paper one.i,ve got the form and filled it in also i have down loaded the medical forms and filled them in .can anyone tell me if i send both forms off together to the same address ,its just that, the medical one says return it to drivers medical group so i,m unsure as to where to send it.i wondered if anyone else had this problem and knew which on to use take care everyone xxxx
