Driving licence question

hi everyone when i was dx 5 years ago i gave up driving now i want to start again.i have to also apply for a photo licence too.unfortunayely i never did get round to telling the dvla about my ms.anyway i,m filling out both the licence renewl form and the medical formi downloaded online.i just wondered do i send both forms back to the same address as i,ve noticed the medical form says to send it to the drivers medical group.i was wondering if any one else had sent their forms in and if so where to.?hope you all have a good weekend xxx
