Hello. I am new to the forum. My wife Carol has SPMS and has suffered for decades. She has suffered from trigiminal Neuralgia since long before COVID and has tried all the nerve painkillers to no avail. She is currently sitting crying with pain, not very pleasant for someone of 66 years. She is on 100mg Tramadol twice daily with 2 50mg tablets mid afternoon. She also takes 1 Carbamazepine at night. The tablets simply do not work and I am asking if anyone has been offered the gamma knife or Botox treatment? Thanks for any assistance.

I found B12 sorted the problem for me and no harm in trying before the more serious options.
“It is important to keep the body healthy to help manage the pain-pathways. Vitamin B Complex, especially vitamin B12, has some evidence in helping regulate nerve function and may be beneficial.”
I feel your pain, as also suffer with Trigeminal Neuralgia and it’s the worst pain to deal with. I was on gabapentin but it wasn’t touching the pain, they then tried me on tramadol with paracetamol at the same time and the combination has eased the crushing feeling in my head and face. It’s worth a try, as know the pain gets you down. I tried b12, b complex didn’t improve any symptoms at all. I’ve found since stopping the gabapentin I’m now dealing with more facial/ head numbness.
