Hi Mini I don’t know where you live but I’ve just completed all my forms again and they went to Blackpool. I don’t know if each area has its own offices but I can give you the address for Blackpool: Blackpool Benefits Centre 1; Warbreck House; Warbreck Hill Road; Blackpool; FY2 0YJ. The telephone number is 08457 123456. I’d try phoning that number and they’d be able to point you in the right direction. Hope this helps.
