I am not sure what “punctuate” means as an adjective (never heard it used this way), but I would take it to mean: “looking like punctuation”, or: “dot-like” (small).
“Non-specific” definitely means it did not have the hallmarks of any particular illness or condition - in other words, nobody has looked at this, and thought: “classic MS” (or classic anything else…)
